What is the formula for corrected calcium?

In conventional units it is measured calcium in mg/dL plus 0.8 × (4.0 − albumin in g/dL). In SI units it is calcium in mmol/L plus 0.02 × (40 − albumin in g/L). Both are the same correction expressed in different units, and both come from Payne's 1973 regression.

Why does low albumin lower the calcium reading?

A standard total calcium assay measures both the free calcium and the calcium bound to protein. Since albumin carries most of the bound fraction, a patient with an albumin of 2 g/dL can show a total calcium well below the reference range while their free, physiologically active calcium is completely normal.

Is corrected calcium as good as ionised calcium?

No. Ionised calcium measured directly on a blood gas analyser is the reference standard, and several validation studies have shown that the albumin correction misclassifies a meaningful minority of patients. The correction is a screening convenience for when an ionised calcium is not available.

When does the albumin correction fail?

It performs worst in chronic kidney disease and dialysis, in critical illness, in multiple myeloma and other paraproteinaemias, and whenever acid-base status is disturbed, because pH itself changes how much calcium binds to albumin. In those settings measure the ionised calcium instead of correcting.

How do I convert calcium between mg/dL and mmol/L?

Divide mg/dL by 4.008 to get mmol/L, or multiply mmol/L by 4.008 to go the other way. The constant comes from calcium's molar mass of 40.08 g/mol and its divalent charge. This calculator shows both units at once so you do not need to do it manually.

What counts as a normal corrected calcium?

Most laboratories quote roughly 8.5–10.5 mg/dL, or 2.12–2.62 mmol/L, but the exact limits vary by assay and by population. Because of that, the reference range in this tool is a pair of editable fields rather than a hardcoded constant.
